The investigation is intended to provide a complete description of the failure event and is used when multiple failure modes are present, when multiple fractures exist, or when it is unclear which condition initiated the failure. The service is also applied when material characteristics are understood but operating and failure conditions are not fully known. Findings are documented in a written report.
